SCHOOL-BASED PROGRAM CASE MANAGER

REPORTING/COORDINATING RELATIONSHIP: Coordinator of Children Services

SUPERVISORY RESPONSIBILITIES: None

PRIMARY RESPONSIBILITIES:
(1) Process all school-based therapy and KY kid’s referrals throughout the Pennyroyal Region.
A) Contact families to explain services & schedule intake appointments w/ assigned therapist
B) Maintain referral logs for all counties served.
C) Appointment reminder phone calls
D) Send referral updates to referral source (guidance counselor, case manager, etc)
(2) Consult with clinic coordinators to develop appropriate treatment and patient education programs within each clinic.
(3) Survey needs and help develop programs/services for juveniles and their families
(4) Attend FAIR team and other community inter-agency meetings as assigned
(6) The management component of this position is directly responsible to the Coordinator of Children Services
(7) Other duties as required/assigned.

CLINICAL RESPONSIBILITIES:
Case manager will provide the following clinical services for substance involved adolescents and their families.
(1) Manage all program referrals, scheduling with appropriate clinician for services.
(2) Participate in staff conferences.
(3) Work as part of a multi-disciplined team in Clinic as well as serve as liaison between community agencies and Pennyroyal Center.
(4) Consultation and education in the community and with community agencies which may include schools, law enforcement, industry, Community Based Services, and others on both program and case basis.
(5) Develop and implement a marketing strategy to improve community relations and increase referrals from schools, juvenile courts, CDW’s, DJJ, DCBS and Health Departments.
(6) Employee must be willing to work beyond regularly scheduled clinic hours if necessary.
(7) Travel to other clinics will be expected based on the needs of the Center.

MINIMUM QUALIFICATIONS:
Minimum of Bachelor’s degree in social work, psychology, or related field and/or Certified Alcohol and Drug Counselor (CADC) credential or willingness to work toward the CADC credential. Prefer 2 years experience in chemical dependency field. Must have a valid driver’s license, reliable transportation, proof of automobile insurance, and must be willing to transport clients on an as-needed basis. Must pass a Criminal Record Check and provide proof of a recent (less than 1-year-old) TB skin test.
