RECORDS CLERK

REPORTING/COORDINATING RELATIONSHIP: Office Manager

SUPERVISORY RESPONSIBILITIES: N/A

PRIMARY RESPONSIBILITIES:

(1) Check clients in and out for appointments.
(2) Prepare schedule and client appointment records.
(3) Data entry.
(4) Serve as back-up to the receptionist when necessary.
(5) Maintain medical record files.
(6) Collect fees for service.
(7) Assist in preparation of medical records for microfilming and keep chart volumes updated.
(8) Copy and send out reports as needed.
(9) Perform reminder calls to all established clients to help decrease FTA’s.
(10) Update client demographic information and ensure accuracy of Insurance.
(11) Scan documents into the client’s Electronic Medical Record.
(12) Assist with Utilization Review process as needed.
(13) Print KASPER reports as needed for APRN.
(14) Assist Clinical Staff as needed.
(15) Other duties as required/assigned.

MINIMUM QUALIFICATIONS:

High school graduate; proficiency in Word Perfect (word processing) and machine dictation. Proficiency with personal computers. Technical experience/interest sufficient to operate computer related hardware, i.e., multiple printers, tape drives, modem communications
