REPORTING/COORDINATING RELATIONSHIP: Coordinator of Community Support Programs



This employee will provide community support services to adult clients with a severe mental illness or co-occurring disorder.

Duties include:
1. Provide and assist in activities that are intended to assure successful community living through utilization of skills training as identified in the individual’s treatment plan.
2. Services should include at least one of the following services: Skills training, cueing or supervision as identified in the individualized treatment plan; Medication adherence and recognizing symptoms and side effects; Non-clinical but therapeutic behavioral intervention, support and skills training; Assistance in accessing and utilizing community resources; Emotional regulation skills; Crisis coping skills; and Developing and enhancing interpersonal skills.
3. Services can include teaching and modeling such skills as the following: routine household care and maintenance; activities of daily living, including personal hygiene; shopping; money management; medication management; socialization; relationship building; participation in community activities; and goal attainment.
4. Community Support Services may be provided in the client’s home or in other community locations. Services are limited to no more than twelve (12) units (3 hours) per day, per client.
5. Assist in accessing housing for identified clients.
6. Must be able to establish rapport with clients and families.
7. Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community advocacy agencies.
8. Employee is also responsible for record keeping associated with the services provided.
9. May fill in from time to time during the absence of other staff, as assigned by supervisor.
10. Attend supervision no less than one (1) time every two (2) weeks.
11. During each subsequent year of employment, a community support associate shall complete and maintain documentation of a minimum of six (6) hours of training or continuing education.
12. Other duties as assigned/required.


High school diploma or equivalent with at least 1 year of full-time experience in working with individuals who have a mental illness or co-occurring mental health and substance use disorder. Must have a valid driver’s license, reliable transportation, and be willing to transport clients daily. Must present proof of automobile insurance and pass a Criminal Record Check. Must present proof of a recent (less than 1-year-old) TB skin test. Within 6 months of employment, this employee must attend the Community Support Associate training.
