REPORTING/COORDINATING RELATIONSHIP: Coordinator of CSP/SMI Case Management

SUPERVISORY RESPONSIBILITIES: None

PRIMARY RESPONSIBILITIES:

This employee will provide case management services to adult mentally ill clients.

Duties include:
(1) Provide community support services to identified clients.
(2) Services should include at least one or more of the following services: as identified in the individualized treatment plan; medication adherence, recognizing symptoms and side effects non-clinical but therapeutic behavioral intervention, support and skills training, referrals to and accessing internal and community resources, emotional regulation skills, crisis coping skills, developing and enhancing interpersonal skills.
(3) Assist in developing housing for SMI clients.
(4) Must be able to establish rapport with clients and families.
(5) Must have transportation and be willing to assist clients in keeping scheduled appointments with internal and external agencies.
(6) Employee is also responsible for record keeping associated with the services provided.
(7) May fill in from time to time during the absence of other staff, as assigned by supervisor.
(8) Timely documentation of treatment plans and required daily documentation.
(9) Attend supervision 1 time every two weeks.
(10) A community support associate shall complete and maintain documentation of a minimum of six (6) hours of training or continuing education.
(11) Other duties as assigned/required.

MINIMUM QUALIFICATIONS:

Bachelor’s degree in psychology, social work, or Human Services field with at least 1 year of experience in working with severely mentally ill adults. Must have a valid driver’s license, reliable transportation, and be willing to transport clients on a daily basis. Must present proof of automobile insurance and pass a Criminal Record Check. Must present proof of a recent (less than 1-year-old) TB skin test. Within 6 months of employment, this employee must attend the Kentucky Level I case managers training.
