COMMUNITY LIVING SUPPORT SPECIALIST/PERSONAL ASSISTANT, IDD

REPORTING/COORDINATING RELATIONSHIP: Director of IDD, Coordinator, General Fund Services/Program Manager

SUPERVISORY RESPONSIBILITIES: N/A

PRIMARY RESPONSIBILITIES:
The IDD Community Living Support Specialist/Personal Assistant is responsible for providing services for assigned IDD clients. The job responsibilities will vary depending on the need at any given time.

(1) IN-HOME TRAINING: Responsible for assessing, writing objectives, training and documenting In-Home Training Services to designated In-Home clients according to frequency listed on service plan. In-Home client may be trained in any or all of the following: self-care, sensory-motor, communication, and home independence (e.g. cooking, laundry, housekeeping, etc.).

(2) LEISURE ACTIVITIES: Provide leisure time services for clients referred by the Case Manager. Involves taking a group of clients to community activity sites, including College for Living. If less than three clients will be served at a time, this must be approved by the Program Manager for IDD Home Services.

RESPITE: Provide temporary relief for IDD clients and their caregivers by either staying with the client in the client’s home or taking the client to activities within the community. Work with Case Manager to establish regular schedule of respite. Document respite activities and billing.

COMMUNITY HABILITATION: Duties include: (1) Transport assigned client to and from designated school or other site. (2) Following Individual Habilitation Plan objectives taken from the school’s Individual Education Plan, the Community Habilitation Provider will provide instructional services and document progress.

MINIMUM QUALIFICATIONS:
Must have a high school diploma or GED and be at least 18 years of age, or be at least 21 years of age and have effective communication skills. Must have valid driver’s license, automobile insurance, and reliable transportation (car that seats 4 comfortably). Must have physical ability to position, turn, and transfer person in wheelchair. Must pass required training, provide proof of a recent (less than 1-year-old) TB skin test, and pass a legally required Criminal Record Check and Sexual Offender Registry check.
