REPORTING/COORDINATING RELATIONSHIP: Program Manager

SUPERVISORY RESPONSIBILITIES: N/A

PRIMARY RESPONSIBILITIES:

This employee will provide case management services to adult males residing in the Pennyroyal Veterans Center. Assignments will be based on client needs.
The Case Manager will work with the client in accessing, coordinating, and monitoring necessary services to assist that individual in stabilizing in the community at his highest potential.
The multiple roles of the Case Manager will include the following:
(1) Individual assessment of:
(A) Basic needs such as shelter, food, clothing, entitlements, etc, for successful transition to self-sufficiency.
(B) Specialized needs such as substance abuse treatment services, rehabilitation, social opportunities, vocational and employment capabilities, and psychiatric services.
(C) Develop a life skills curriculum and facilitate classes to help clients prepare for post-program living.
(D) Patient needs for transportation in order to access services.
(2) Provide ongoing contact with community agencies necessary to develop additional resources and coordinate services.
(3) Monitoring service delivery.
(4) Client advocacy.
(5) Assist clients with the problems they face in early recovery, especially legal issues, housing and employment.
(6) In collaboration with other staff and assistant staff, initiates and maintains complete up-to-date client records including but not limited to client files, shift logs, incident reports, and discharge summaries.
(7) Ensures maintenance of clinical information in staff-only files and maintains client confidentiality.
(8) Collaborates with other Program Coordinators and facility staff in program development and client consultation. Provides counseling, feedback, and education to all clients in collaboration with other members of the management/treatment team.
(9) Willing to have on-call responsibilities.
(10) Assist with clients budget and monitor budget compliance.
(11) Monitor SNAP application process.
(12) Other duties as assigned/required.

MINIMUM QUALIFICATIONS:

Minimum of Bachelor’s degree in social work, psychology, or related field. Prefer 1 year experience in chemical dependency field. Must have a valid driver’s license, reliable transportation, proof of automobile insurance, and must be willing to transport clients on an as-needed basis. Must pass a Criminal Record Check and a Sexual Offender Registry check, and provide proof of a recent (less than 1-year-old) TB skin test.
