CASE MANAGER, SMI

REPORTING/COORDINATING RELATIONSHIP: Coordinator of CSP/SMI Case Management

SUPERVISORY RESPONSIBILITIES: None

PRIMARY RESPONSIBILITIES:

This employee will provide case management services to adult mentally ill clients.

Duties include:
(1) Provide intensive case management to identified clients.
(2) Have ongoing contact with community agencies necessary to develop additional resources and coordinate services.
(3) Assist in developing housing for SMI clients.
(4) Must be able to establish rapport with clients and families.
(5) Must have transportation and be willing to assist clients by providing outings and keeping scheduled appointments with various community advocacy agencies.
(6) Employee is also responsible for record keeping associated with the services provided.
(7) May fill in from time to time during the absence of other staff, as assigned by supervisor.
(8) Other duties as assigned/required.

MINIMUM QUALIFICATIONS:

Bachelor’s degree in psychology, social work, or related field with at least 1 year of experience in working with severely mentally ill adults. Must have a valid driver’s license, reliable transportation, and be willing to transport clients on a daily basis. Must present proof of automobile insurance and pass a Criminal Record Check. Must present proof of a recent (less than 1-year-old) TB skin test. Within 6 months of employment, this employee must attend the Kentucky Level I case managers training.
