The position will provide crisis case management, crisis intervention and response for adults with IDD and Pre Admission Screening and Resident Review (PASRR) case management. The case manager will be responsible for developing case plans and developing a Person-Centered Plan. This position will provide coverage for all counties in the Pennyroyal Region.

Duties include:
1) Provide home/Crisis Services Program visits to clients in order to monitor their overall functioning in their environment. This will include ongoing assessment, observation, and monitoring mental status and medication.
2) Intake on new PASRR referrals to include developing a yearly Plan of Care, monthly face to face visits in nursing homes and maintaining documentation of all visits. The case manager should be able to guide the writing of goals and objectives to ensure clarity, measurability and applicability to the client.
3) Complete all required documentation for the crisis program and PASRR program.
4) Educate and assist all clients and their families regarding the client’s treatment and care.
5) Provide training and back-up support for Therapeutic Aides.
6) Be available for on call for Pennyroyal Region for crisis case management intervention and be available to provide backup coverage in emergency situations.
7) Be knowledgeable of service providers and referral procedures.
8) Be knowledgeable of the different funding sources for case management and the services/equipment commonly used to include regulations for waiver programs and state general funds.
9) Develop Person-Centered goals for all clients served.
10) Assist supervisor with monitoring services within the community including being able to attend Plan of Care meetings, trainings and home visits as needed.
11) Other duties as required/assigned

Minimum of Bachelor’s degree in psychology, social work or related field. Experience in the mental health field preferred.
Must have a valid driver’s license, reliable transportation and present proof of automobile insurance as required by the State of Kentucky and be able to travel throughout the region. Must pass a Criminal Record Check, a Child Abuse and Neglect Check, A Nurse Aide Abuse Registry check and a Sexual Offender Registry check. Must present proof of a recent (less than 1 year old) TB Skin test. Must be available 24 hours a day, 7 days a week to work a flexible schedule as required by program needs.
